Found a wild one! In Liurnia, north of the Academy Gate Town, there's a seemingly empty cliffside. If you attack the air at the exact spot where the "ghost" jellyfish are floating at night, an invisible footbridge appears! Leads to a cave with a somber smithing stone [7]. The map showed NOTHING.
